Post-Surgery Knee Replacement: The First 30 Days of Recovery at Home

• • 2 min read

Undergoing a Total Knee Arthroplasty (TKA) is a major step toward a pain-free life. However, surgical success is only half the battle—the remaining 50% depends entirely on disciplined, guided post-operative rehabilitation in your first 30 days at home.

Week 1: Swelling Control and Early Quadriceps Activation

In the first seven days post-discharge, your primary clinical goals are pain management, swelling reduction, and waking up the quadriceps muscles:

  • Ice and Elevation: Keep the operated leg elevated above heart level with pillows placed under the calf and ankle (never directly under the knee joint, which can cause flexion contracture).
  • Ankle Pumps: 20 to 30 repetitions every hour to promote venous return and prevent Deep Vein Thrombosis (DVT).
  • Static Quad Contractions: Gently pressing the back of the knee into the bed surface to activate the vastus medialis.

“Never place a pillow directly beneath your healing knee. Keeping the knee straight while resting prevents permanent loss of terminal knee extension.”

Week 2 to 3: Gaining 90° Flexion and Safe Walking Mechanics

By the second week, wound staples or sutures are typically checked, and active-assisted range of motion expands. Under the guidance of your home physiotherapist, you will focus on:

  • Heel Slides: Smoothly sliding the heel toward your buttocks to achieve 90 degrees of knee flexion.
  • Straight Leg Raises: Lifting the straightened leg 6 to 8 inches off the bed without a quadriceps lag.
  • Walker-to-Stick Transition: Moving from a two-handed walker to a single elbow crutch or quad cane as weight-bearing tolerance improves.

Week 4: Functional Balance and Stair Climbing

By day 30, your walking pattern should be smoother, and home visits focus on functional daily activities: step-ups, gentle mini-squats with support, and negotiating household stairs using the clinical rule: “Up with the good leg, down with the operated leg.”

Why Home Physiotherapy Accelerates Healing

Traveling to an outpatient clinic with a freshly operated, swollen knee causes unnecessary discomfort and joint irritation. In-home physiotherapy allows precise monitoring of swelling, gait training on your actual flooring, and ergonomic adjustments to your bed and bathroom.
